LTC Exit Codes

Exit reasons are one of the pieces of information that will be used to build up a picture of the changes to the LTC Service patient population. They will also be used for audit, and eventually will form part of the information in the line by line reporting provided to pharmacy for their LTC Service Fee payments. You will be advised if there are any changes to the exit reasons.

LTCX1 | Adherent | The patient is now adherent |
LTCX2 | Moved to ARRC | The patient has moved permanently to an ARCC facility |
LTCX3 | Moved Pharmacy | The patient has moved to another LTC pharmacy provider |
LTCX4 | Patient Request Exit | The patient no longer wants to receive the service |
LTCX5 | Deceased | The patient is deceased |
LTCX6 | No Rx in 3/12 | The patient has not been dispensed a pharmaceutical in the last 3 months |
LTCX7 | Core at Another phcy | The patient has received Core Pharmacy Services from another Provider/s on a regular basis |
LTCX9 | Registered in Error | The patient has been incorrectly registered |
